Information about become a soccer referee in Broadview Heights

Soccer referees in Broadview Heights must be 14 years of age and must complete the required forms listed below. Soccer referees should have experience playing soccer and be knowledgeable about the rules and the game.

You can begin the process of becoming a soccer referee in Broadview Heights by submitting an employment inquiry form to the Athletics Manager,via email Gary Diehl.

Referee Pay Card

Submit your pay card after you have refereed your games. Checks will arrive to your mail approximately 2 weeks after submission of the card. Card should be submitted at the Rec Center front desk.

Each referee is required to have the following forms on file with us: Independent Contractor Form: Soccer (PDF)

(This is only required once) W9 (PDF)

(This is only required once) OPERS Form (PDF)

(This is only required once) Minor Work Permit (PDF)

(This is required for each school year per activity) State of Ohio Concussion Training Certificate
